According to Loughman et al. (2011) this species appears to be endemic to the Greenbrier River basin in the Valley and Ridge province of West Virginia. Within the Greenbrier system it occurs primarily in tributaries to the Greenbrier mainstem, with stable populations in the East and West Fork, and Thorny, Knapp, and Deer creeks.
Ecology & Habitat

Habitat

Cambarus smilax occurs in tributaries and headwater portions, as well as middle and lower portions of the Greenbrier River and is lacking from larger reaches of the river. It occurs under and among boulders and slabs from April through August and utilizes leaf packs and other depositional microhabitats from September through November. Its winter habitat is unknown (Loughman et al. 2011).
